The Department of Social Services collects your name, phone number and contact address via the Request DSS Data Portal, hosted by Aristotle Metadata, to manage your request for data, or otherwise as permitted or required by law. We may disclose your personal information to other parties where the disclosure is required or authorised by law. If you do not provide your name and contact address we will not be able to consider your request for data.

Your password can’t be too similar to your other personal information.
Your password must contain at least 8 characters.
Your password can’t be a commonly used password.
Your password can’t be entirely numeric.

Password confirmation Enter the same password as before, for verification.
